I had completed a trauma fellowship at one of the busiest penetrating trauma centers in the U.S. and thought I was ready for anything; mistake number one. Fortunately you have at least several days of overlap with the outgoing group of surgeons that you are replacing. I first met the surgeon I was replacing in a dusty tent in Tikrit, Iraq. Luckily for me it was Marty Schreiber, one of the best trauma surgeons I know.

“Philosophical Issues in Psychiatry IV”
All the chapters of this book began as talks given at a conference held in Copenhagen, Denmark, on September 9– 11, 2015. The conference was organized and chaired by the editors of this volume, Kenneth S. Kendler (KSK) and Josef Parnas (JP). The conference itself was a stimulating three- day experience. Each talk was followed by a formal commentary and then an open give- and- take with the audience and other speakers, who were rarely shy about raising hard questions. So each speaker coming to the meeting agreed to present a paper of their own, comment formally on another, and write them both up for publication. As with the earlier volumes in this series (Kendler and Parnas 2008, 2012, 2015), we have sought to capture the interactive nature of the meeting through the organization of this volume. In addition to a general introduction, written by KSK, each individual paper has an introduction (written by KSK or JP) and a discussion.

cardiologists and anesthesiologists preparing for this certification process. At the time of its writing, there were no other published works available that comprehensively dealt
with the material covered in these examinations in a question, answer, and discussion format. This second edition is thoroughly revised and 100 questions have been added. The authors have used this format in teaching echocardiography to cardiology fellows in training. One of the main impetuses for initiating this work was the request by many of the trainees and prospective echocardiography examination candidates to write such material. Similar requests have also come from echocardiography technicians preparing for their certification examination. There are 500 well-thought-out questions in this review book.

CHEMORADIATION IN CANCER THERAPY
As the treatment of cancer continues to evolve, clinicians are constantly seeking new and innovative strategies to expand the use of currently available treatment modalities. Among the different strategies to improve therapy, the combining of chemotherapeutic drugs with radiation has perhaps had the strongest impact on current solid tumor treatment practice. This combination has been in use for many decades, but now has become a common treatment option in many clinical settings. This is particularly true for concurrent chemoradiotherapy, which in many recent clinical trials has been shown to be superior to radiotherapy alone in controlling local–regional disease and in improving patient survival. Combining chemotherapeutic drugs with radiotherapy has a strong biologic rationale. Such agents reduce the number of cells in tumors undergoing radiotherapy by their independent cytotoxic action and by rendering tumor cells more susceptible to killing by ionizing radiation.

Today, genetics, informatics, imaging and integrative technologies, such as nanotechnology, are redefining our understanding of the body, health and disease; at the same time, health is no longer simply the domain of conventional medicine, nor the clinic. The ‘birth of the clinic’ heralded the process through which health and illness became increasingly subject to the surveillance of medicine.

The motivation to write Forensic Alcohol Test Evidence (FATE) is an extension of my long-standing interest in learning and explaining how things work. As an undergraduate, my focus on the neurophysiology of the brain and behavior was a passion that grew, as did the opportunity to work with excellent mentors at Queens College of the City University of New York and at Rockefeller University, where I continued working after completing my undergraduate degree and before starting my graduate work in psychobiology at Binghamton University. My research at the Rutgers University Center of Alcohol Studies and the Rutgers Alcohol Behavior Research Lab allowed me to
follow my scientific curiosity about the relationship between the brain, behavioral
psychology, and alcohol. Coincidentally, this academic background in both laboratory research and clinical testing related to alcohol met the need of the legal community to have a resource to answer both analytical and biobehavioral questions about the role, if any, of alcohol in a crime or accident.

The incidence of obesity, type 2 diabetes mellitus, hypertension, dyslipidemia, and
coronary heart disease, which form the core components of the metabolic syndrome,
is increasing at an alarming rate in almost all countries. The incidence of the metabolic syndrome is especially high in the Indian subcontinent. This increase in the incidenceof the metabolic syndrome has been attributed to lack of exercise, increase in the consumption of energy-dense food, and environmental changes. What is more disturbing is an enormous increase in the incidence of obesity in children, as a result of which these children are likely to develop other features of the metabolic syndrome as they grow to adulthood.

The Ischemic Penumbra
In the world of acute ischemic stroke, nothing is more important than an understanding of the ischemic penumbra. The whole concept of the existence of salvageable tissue, that is, the penumbra, is based on the daily clinical observation of almost immediate and early recovery from an episode of hemiparesis or other clinical expression of occlusion of a blood vessel within any of the arterial territories of the brain. Indeed, even before there was an appreciation of the complexities of the ischemic cascade, many clinicians dealing with stroke understood that something akin to the ischemic penumbra was likely to exist. These ideas, which fueled pivotal experimental studies to document the penumbra, clashed with the then prevailing dogma that the ischemic brain was doomed to die within minutes of onset of the clinical event. It is because of this central positioning of the penumbra in stroke outcome and therapy that we felt that a volume dedicated to its existence was timely.

quality of life of our patients, orthostatic intolerance, sweating disorders, sexual dysfunction). In some cases, they may be harmful (syncope with falls, heat

Diagnosis of ANS disorders is stillu underrepresented,despite their common occurrence in clinical practice.
